Send the referral
Complete the referral form below with your provider information and the general reason for referral. Our team will contact your office to gather patient information through an appropriate channel. You can also call (720) 432-0958, fax (720) 432-0978, or have your patient contact us and mention you. Email is for general referral questions only; do not include patient information.

We evaluate promptly
Your patient receives a comprehensive psychiatric evaluation with Dr. Wagner. We verify benefits and handle prior authorization before any treatment begins.

We treat, and keep you informed
With your patient’s consent, you receive our findings, the treatment plan, and progress updates measured with validated scales.

Your patient returns to you
When the course of treatment ends, care transitions back with a clear summary and maintenance plan, or we co-manage long-term where that serves the patient best.

Frequently asked questions
Do you take over care, or co-manage?
Whichever serves the patient. For interventional courses like TMS or Spravato®, we typically treat the referred problem and return the patient to you with a summary and maintenance plan. For complex medication management, we’re glad to co-manage alongside your therapy or primary care.
How do I get records and updates?
With the patient’s written consent, we send evaluation findings, the treatment plan, and progress updates directly to your office. We’re also happy to talk clinician-to-clinician by phone.
Which patients are not a fit?
We’re an outpatient clinic, so patients in acute crisis need emergency services (911, the ER, or 988) first. We are also not in-network with Medicare or Medicaid, and state regulation prohibits us from seeing Health First Colorado (Medicaid) members even on a self-pay basis, so Medicaid patients need a Medicaid-enrolled provider. If we’re not the right fit, we’ll say so quickly and suggest alternatives when we can.
Does insurance cover the advanced treatments?
Most major commercial plans cover TMS and Spravato® for treatment-resistant depression once step-therapy criteria are met, typically documented medication trials that your records help establish. We handle the prior authorization and give the patient a written estimate first.
